It’s fine, your OPSEC isn’t blown just for using the QR code method, that’s literally an official feature built into Tor to make grabbing bridges easier when you don’t wanna type them out manually. The QR code just encodes the same bridge lines you’d normally copy-paste from Tor’s site or email, nothing special or traceable about it beyond the normal bridge request itself.

If it worked once then stopped, that’s probably just the bridge itself being unstable or already blocked, not a sign you got compromised. You can always request new ones through the same QR method or through email/torproject’s site, and it’s good practice to rotate bridges every so often anyway. If you’re worried, just nuke the old file from persistent and grab fresh bridges, but nothing about scanning that QR code put you at extra risk.
